Charges on this page apply to
subscribers and non-subscribers both.
1. Applicable state and local taxes are collected in addition to posted prices.
2. Limit 10 service items per subscriber per month. Thereafter, non-subscriber pricing applies.
3. The “phlebotomy” and “swab” charges cover labor for collecting a blood or tissue specimen. Separate charges apply to any laboratory analysis of the specimen.
4. The “administration” and “injection” charges cover labor only. Separate materials charges apply for any medications, vaccines, or fluids administered.
5. After-hours meetings, house calls, and hospital calls are subject to availability.
6. Most subscribers choose to pay monthly, with subscription charges assessed on the final day of each calendar month. Charges for partial months are prorated. Other payment periods are available, for example: weekly, every-two-weeks, quarterly, etc.
7. Listed “subscription” charges apply to new subscribers. Charges for established subscribers may differ, according to previous Subscription Agreements.
8. The monthly “Controlled Substance Agreement” charge includes a subscription as well, regardless of age.
9. Hourly charges are rounded to the nearest one-tenth of an hour.
10. Charges for various “medications,” “vaccines,” and “other send-out lab tests” are too numerous to list here. Specific estimates are available upon request.
11. Lab tests are offered only in conjunction with physician’s evaluation and management.
12. For subscribers with Controlled Substance Agreements, a price of $0 applies to unannounced urine drug tests.
13. “Medical records” charges are regulated by the Minnesota Department of Health.
14. Both “per-chart” and “per-page” charges apply to each medical records request.
15. The “credit card convenience surcharge” is not greater than our cost of acceptance.
16. The “E-check convenience surcharge” applies to each payment.
17. Increased “missed appointment” charges apply to repeated occurrences.
18. The “late payment” charge applies to monthly subscription charges not paid on (or before) the final day of each calendar month.
